-Cheri Aungst notary - Avis

You can also print the form from our Crimewatch page under "forms" and return it as described below.

Please call the department at 570-753-5672 to make arrangements to return the completed application along with the requested documents listed on the application.  If you would like an application emailed to you please send an email to pinecreekpd@comcast.net and put "Operation Christmas" in the subject. The application will need printed and returned in person.
